Your Rights and Responsibilities

Sunset Healthcare fully complies with all applicable federal civil rights laws and does not discriminate on the basis of sex, race, color, religion, ancestry, national origin, sexual orientation, disability, medical condition, marital status, or registered domestic partner status. Patients and their representatives have the right to be informed of the patient’s rights in a language and manner that is understandable to them. As your hospice provider, we are dedicated to protecting and promoting the exercise of these rights.

As a patient of Sunset Healthcare, you have the right:

  • To exercise your rights as a hospice patient.
  • To be treated with consideration, respect, dignity, and individuality, including privacy in treatment and personal care; and to have your property treated with respect.
  • To be free from verbal, mental, sexual, and physical abuse, including injuries of unknown origin, neglect, and misappropriation of property.
  • To receive effective pain management and symptom control.
  • To actively participate in the development of your hospice care plan.
  • To refuse care or treatment at any time.
  • To choose your attending physician.
  • To have a confidential clinical record and to access or request the release of information in your clinical record in accordance with the HIPAA Privacy Rule and as outlined in our Notice of Privacy Practices.
  • To receive clear information about the services covered under the hospice benefit.
  • To receive detailed information about the services that Sunset Healthcare will provide and any specific limitations of those services.
  • To have 24/7 access to nursing services by contacting our office.
  • To know the name of the person responsible for supervising your hospice care, and how to contact them during business hours and in emergencies.
  • To make informed health care decisions, including the acceptance or refusal of certain medical treatments, and to exercise your right to Advance Directives. We will explain your options and provide you with a copy of our Hospice Policy on Advance Directives.
  • To be informed orally and in writing about:
    • The extent to which payment for hospice services may be expected from Medicare, Medicaid, or any other federally-funded or federal aid program known to Sunset Healthcare.
    • The charges for services that may not be covered by Medicare, Medicaid, or any other federally-funded or federal aid program known to Sunset Healthcare.
    • Any charges you may be responsible for before care begins, and any changes to these charges when they occur.
  • To be informed about accessible auxiliary aids and language services and how to access them.
